We recommend that you arrive a bit early for your first appointment to get settled, meet our team, and complete a few forms. It will speed up your new patient intake if you could bring the following information with you:
- Your doctor’s name and contact info
- A list of your current prescriptions
- Any information concerning allergies or existing medical conditions
- Your dental health insurance information, if any

Payment is due when services are rendered, unless other arrangements have been made. If you have dental coverage, we are happy to submit your insurance claim electronically and to accept the assignment of benefits. We are also pleased to assist
with insurance verification, billing, and claims, ensuring a smooth and hassle-free experience. Please remember, you are fully responsible for all fees this office charges, regardless of your insurance coverage.
